Children and youth that meet the criteria for specialized health care program must have a medical condition that requires specialized in-home health care treatment intervention.
The CSKF specialized health care services provide trained resource families to provide health maintenance and preventive health care in accordance with their specialized training. The medical skills of this resource families are needed to meet the acute or chronic health condition requiring specialized medical treatment to remain safely in placement.
Specialized health care needs program or otherwise known as medically fragile program requires the direct intervention of a multidisciplinary team of social worker, nurse, case manager, physicians and allied health care providers. CSKF is dedicated to meeting the medical/mental and developmental needs of clients with special health care needs.
